Those services include but are not limited to providing support around child development and trauma as it relates to parenting skills, life skills, relationship skills, behavior management skills to assist children and families during periods of transition and crisis, ultimately assisting children in achieving stability and permanency.
Afterhours Support Worker provides direct care services to children and families in a variety of settings, services including, but not limited to, crisis management, de-escalation, behavior management, relationship skills; life skills and parenting skills ? Support case plan goals through teaching, role modeling, and demonstration.
? Develops short term safety plans to resolve crisis situations.
? Works with community partners to access supports for children and families during periods of crisis and transition Afterhours Support Worker frequent communication/coordination with afterhours care coordinator during crisis situations.
? Supervises and transports children and families, as needed.
? Documents information and completes all necessary Saint Francis Ministries forms according to timelines.
? Afterhours Support Worker assists with providing accurate and thorough information to add to placement referrals ? Supports resource families to maintain placements and help children work toward permanency.
? Monitors and supervises family interactions.
? Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
? Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
? Provides clerical support to case teams as needed.
? Assists with filing or closing out files as need or requested by Supervisor.
? Implements Saint Francis Ministries policies and follows directives as required.
Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules.
? Afterhours Support Worker ensures clients' rights are protected.
? Afterhours Support Worker is knowledgeable of and follows all safety procedures.
